CREATING FORMS

Forms can be used in the platform to build and publish online forms, surveys, and assessments. Each of these types have their own approval workflows and statuses.

Forms/Surveys can be set to Completed/Approved/Not Approved, while assessments can be Competent/Not competent/Marking Required. If marking is required, an administrator has the capability to mark specific questions as competent after reviewing the submission. Once enough questions have been set to competent, the assessment itself is set to a Competent status.

 

1. Navigate to ADMIN FORMS.

2. Click on ADD NEW FORM.

3. Enter the form details in the SETTINGS tab.

Note: Only choose the ASSESSMENT option if this form is going to be used as an assessment within a course.

4. Click the BUILD FORM tab.

5. Drag and drop the desired question type to the main area. Click EDIT on the question to set it up.

6. Enter question title.

7. If mandatory, set “is required?” to TRUE.

8. If multiple choice, expand the CHOICES tab and click on FAST entry.

9. Enter the options (one per line), and click OK.

10. Repeat steps 3-7 until all questions are set.

Note that if it is an assessment question with a correct response, you can choose the correct answer by clicking on the button on the right-hand side for each required question, and defining the correct response.

If it is a COMMENT question type which requires manual marking by an administrator because they need to review the learner's response, type “marking required” in the CORRECT ANSWER box.

11. Click the PERMISSIONS tab to allocate the form to users

(Note that if this form is being used as an assessment, skip the allocation of permissions.)

12. Set the required permissions and click APPLY PERMISSIONS.

13. Click SAVE once complete and navigate back to the ADMIN FORMS page.

14. For the desired form, click PUBLISH from the ACTIONS drop-down button.

This form is now ready to be visible in the MY FORMS page, or to be selected in the activity list if being used as an assessment.
