Forms are powerful tools on our platform that enable you to effortlessly create and deploy online forms, surveys, and assessments. Each form type has distinct approval workflows and statuses tailored to your administrative needs. For Forms and Surveys, administrators can easily manage their status by marking them as either Completed, Approved, or Not Approved. Assessments offer a more nuanced set of statuses, allowing you to designate them as Competent, Not Competent, or Marking Required. In instances where marking is required, administrators can review submissions and mark specific questions as competent. Once a sufficient number of questions attain competence, the overall assessment is elevated to a Competent status. We have created a guide to help you harness the full potential of forms within Learnt. This guide will walk you through the step-by-step process of streamlining your administrative tasks with confidence.

Toolbox

The Toolbox contains the question and panel types described below. Drag and Drop the desired question type into the editor section to start building your form/assessment.

Single Input

Learners enter their textual, numeric, or date and time answers into a single-line editor. Use the Single-Line Input type for questions that require short answers.

Checkbox

Learners click one or several checkboxes to select answers. Use the Checkboxes type for questions that accept multiple answers.

Radiogroup

Learners use radio buttons to select a single answer. Use the Radio Button Group type for questions that can have multiple options but accept only one answer.

Dropdown

Learners select a single answer from a drop-down list. Like Radio Button Group, the Dropdown type can be used for questions that have multiple options but accept only one answer. However, the Dropdown UI can display more options while occupying less screen space.

Comment

Learners enter their lengthy answer into a resizable multi-line text area. Use the Comment type for open-ended questions that accept multi-line answers.

Rating

Learners select one number (or value) within a range. Use the Rating Scale type when you want Learners to enter a rating.

HTML

Use the HTML type to format text as needed, include links, and insert media or other custom elements into the survey. This type is used for presentation only and does not produce a value to be saved in the survey results. Be aware that HTML content can be vulnerable to security breaches. Ensure that the links you insert lead to trusted resources.

File

Use the File Upload type to allow Learners to upload files.Maximum file size can be defined in bytes (optional) using the question settings.

Panel

The Panel type is a container for other questions and panels. Use this type to group several questions or panels and control them all together.

Date Picker

Learners can enter numeric and forward slash characters only in order to create a date.

Tag Box

Learners select an answer from a drop-down list. Unlike Radio Button Group, the Dropdown type allows Learners to select more than one option, similar to the check box style.

Signature

Learners use mouse or touch gestures to draw their signature within the allocated input area. Use this type to obtain the respondent's signature or any other hand-drawn input.

Step by Step Guide 

1. Navigate to ADMIN FORMS, and click on the ADD NEW FORM button.

2. In the FORM SETTINGS, enter the required information:

Name

The title of the form

Form/Assessment radio button

Choose whether the form is a standard “form” (eg web form, survey etc), or a quiz assessment. The main difference between these options is that an assessment requires a pass mark, and the submission status terminology differs for forms (completed, approved, awaiting approval) versus assessments (completed, marking required, not competent). Assessments should only be used if this is to be used as an activity within a course.

Category (Optional)

A category filter that will become visible in the MY FORMS page. Not required for assessments.Learners can narrow down forms displayed by using this filter.

Description (Optional)

A descriptor of the form. Not required for assessments.

Tags (Optional)

A tag filter that will become visible in the MY FORMS page. Not required for assessments. Learners can narrow down forms displayed by using this filter.

Show In Forms Page Checkbox

If this option is ticked, the form will be visible to learners in the MY FORMS page, assuming publish and permission statuses are met. Not recommended for assessments.

Anonymous Submission Checkbox (Optional)

Learners can submit forms without their name recorded against the submission. Not recommended for assessments.

Sign Off Required Checkbox (Optional)

This should be selected for forms (not assessments) which require a sign off before being set to complete/approved status.

Do Not Allow Resubmission Checkbox (Optional)

Forces the form to lock after a learners first submission. Not recommended for assessments.

Published Checkbox

When ticked, the form is set to published status, and is visible for selection when creating an activity, or visible in the MY FORMS page (if permissions match).

Start Date/End Date (Optional)

If used, forms cannot be submitted outside of this date range within the MY FORMS page.

Image Thumbnail (Optional)

This image is used for the thumbnail in the MY FORMS page, as well as the default screen for the form in the course player, if it is used as an activity within a course.

 

  1. Click the BUILD FORM tab to add questions. Drag and Drop using the question types from the toolbox (information relating to specific question types in next section).
  2. For specific questions, click on the EDIT button at the top of the question to enter the:
    1. Name: This is the question displayed
    2. Is Required: Defines whether the form/assessment can be completed without answering the question
    3. Choices/Rate Value: Click FAST ENTRY and enter the response options (where applicable). One option per line.
    4. Click OK to close the edit dialog box
    5. If the question has a correct response, click on the CORRECT ANSWER option on the right hand panel, and select the correct response(s). If it is an open text question which will require administrator review before being set to competent, type “marking required” in this box. This question can only be set to correct by the administrator.
    6. Repeat for all questions.
  3. Click PREVIEW tab to view what the form/assessment will look like.
  4. Click on the PERMISSIONS tab to allocate it to the desired subset of users. Note that this is only required if the form/assessment is visible in MY FORMS page. If it is for an assessment within a course, skip this step.
  5. Click SAVE.
